A 90-Day Plan for Low Hemoglobin

This plan is mostly about absorption, not intake. Most people with low haemoglobin in India are already eating iron — they are drinking chai with it, taking it with milk, and absorbing a fraction. Moving one cup of tea does more than switching supplements.

Week 1 — Test and Rule Out the Cause

  • Ferritin (not just haemoglobin) — it falls first and can be low while haemoglobin reads normal
  • Vitamin B12 — causes a different anemia that iron will not fix
  • TSH

And answer this honestly: are your periods heavy? Soaking a pad every hour or two, bleeding beyond seven days, clots larger than a rupee coin, changing protection at night?

Heavy menstrual bleeding is the most common cause of iron deficiency in Indian women and the most routinely dismissed. No diet keeps up with ongoing heavy loss. If this is you, a gynaecologist is the step that changes the outcome — fibroids, adenomyosis, thyroid problems and PCOS all cause it and all are treatable.

Also see a doctor for: haemoglobin below 9, breathlessness or palpitations, blood in the stool, or craving ice, chalk, mud or raw rice — pica, a classic sign of significant deficiency.

The Two Rules That Decide Everything

1. Vitamin C in the same sitting

Multiplies plant-iron absorption several times over. Lemon on the dal, tomato in the sabzi, amla, guava, orange. Same glass, not the same day.

2. Tea, coffee and dairy away from iron

  • Tea/coffee: one hour either side. Tannins bind iron substantially.
  • Milk, curd, calcium: two hours. Calcium competes directly.
  • Never stir an iron powder into milk.

Also check: antacids and omeprazole reduce the stomach acid you need to absorb iron. Very common and often missed.

The Daily Routine

  • On waking: 1 tsp Hemoglobin Powder + 1 tsp amla in water with lemon
  • Breakfast: poha with plenty of lemon, or ragi porridge with guava. Chai an hour later — this is the hardest habit to change and the most valuable.
  • Mid-morning: 1 tsp beetroot in juice, or fruit
  • Lunch: masoor or chana dal with lemon squeezed over · a green vegetable — drumstick, methi or palak · bajra or jowar roti · salad with lemon. Curd an hour later, not with the meal.
  • Afternoon: 1 tsp spirulina or moringa in orange juice. Then have your chai.
  • Dinner: rajma or chana · drumstick or methi sabzi · lemon on everything

Two Free Things That Work

  • Cook in an iron kadhai — genuinely transfers iron into food, more so with tomato-based dishes. One of the few folk practices with solid evidence.
  • Soak and sprout dals — reduces phytates, which block iron.

Timeline and Retesting

Week Expect
2–4 Possibly slightly more energy. Do not judge by feel.
4–8 Haemoglobin begins moving
Day 90 Retest haemoglobin AND ferritin
Months 3–6 Ferritin rebuilding — the stores take longest

Do not stop when haemoglobin normalises. Ferritin is still empty and it will come straight back. Continue to the six-month mark.

If Nothing Changed at 90 Days

Check the absorption rules first — that resolves most cases. If they were followed properly, the cause needs investigating: ongoing blood loss, coeliac disease, H. pylori, or thalassaemia trait, which is common in some Indian communities, mimics iron deficiency, and does not respond to iron.

Iron deficiency that does not respond to iron needs a diagnosis, not more iron.
